The CKD Model is an AI-powered predictive tool to predict the risk of diabetic patients in CKD stage 1 or 2 to deteriorate to stage 3 and above in the next 5 years.

The CKD Model uses advanced machine learning techniques to identify patients at high risk of function decline, allowing clinicians to deploy early intervention methods to avert or delay kidney function deterioration in their patients.

Integrated into the National Electronic Health Records (NEHR), this model supports clinicians nationwide by providing timely, actionable insights that enable early intervention. Opportunity
There are more than 400,000 patients with diabetes in Singapore, and CKD is one of the most common complications. Rising prevalence of diabetes and CKD highlights the need for early detection and intervention. Early detection of patients at risk can prevent progression to kidney failure and reduce healthcare burdens.
Solution

Synapxe developed the CKD Model, an AI-powered predictive tool to estimate the risk of CKD worsening in diabetic patients within 5 years. The model analyses over 177 variables across sociodemographic, diagnostic, laboratory, and medication history categories to predict the risk of CKD progression to stage 3 or above within five years.

Integrated into the National Electronic Health Records (NEHR) for access by clinicians, the model classifies patients into low, high, and extremely high risk to guide clinical decisions and early interventions. This seamless integration allows for real-time risk assessment without additional data entry, supporting efficient and informed clinical decision-making.

The CKD Model enables clinicians to provide targeted advice and early treatment to slow CKD progression.
