The Assisted Chronic Disease Explanation using AI (ACE-AI) tool is an AI-powered digital assistant designed to support clinicians in chronic disease management by providing personalised risk profiles and health insights for common chronic diseases.
 
Leveraging advanced AI algorithms, ACE-AI predicts the risk of developing diabetes and/or hyperlipidaemia within the next three years. The tool provides primary care providers with an objective and standardised way to identify high-risk individuals by flagging those with over a 75% chance of being diagnosed within the next 3 years.  

To be integrated into clinical workflows to primary care providers in both Healthier SG General Practitioner clinics and polyclinic settings from early 2027, ACE-AI supports doctors by providing timely, objective risk assessments for Healthier SG enrollees. By facilitating earlier detection and management, the tool aims to prevent or delay serious cardiovascular events, improve patient outcomes, and potentially reduce long-term healthcare costs.
