Synapxe developed the Health Discovery (HD) platform during the COVID-19 pandemic to enable remote patient monitoring when off-the-shelf solutions could not meet Singapore’s operational and clinical requirements. The platform later evolved into Health Discovery+ (HD+), which is now used across polyclinics to support the remote monitoring of patients with chronic conditions such as diabetes and hypertension.
According to Synapxe’s Rino Tan, the development and of scaling HD+ nationally required close collaboration with healthcare partners to standardise workflows, train users, and balance consistency with local operational needs. Future improvements would focus on earlier clinical alignment, scalability, and change management.
